Denton Vision Center is committed to protecting the privacy of your health information. This notice explains how Denton Vision Center may use and disclose your protected health information (PHI), and your rights concerning that information.

How Denton Vision Center May Use or Disclose Your Health Information

Denton Vision Center may use and disclose your PHI for the following purposes without your written authorization:

Authorization for Other Uses

Other than the uses and disclosures listed above, Denton Vision Center will obtain your written authorization before using or disclosing your PHI. You may revoke your authorization at any time, except for disclosures already made based on your authorization.

Notification of Breaches

Denton Vision Center will notify you if it becomes aware of any unauthorized access, use, or disclosure of your unsecured health information as required by law.

Your Individual Rights

You have the following rights regarding your PHI held by Denton Vision Center:

  • Request Restrictions: You may request restrictions on how your PHI is used or disclosed. Denton Vision Center is not required to agree but will comply if it does.
  • Confidential Communications: You may request to receive communications of your PHI by alternative means or at alternative locations if disclosure via usual methods could endanger you.
  • Inspect and Copy: You have the right to inspect or obtain a copy of your health information, with certain exceptions. Denton Vision Center may charge reasonable fees.
  • Amend: You may request corrections to your PHI. Denton Vision Center may deny requests for valid reasons but will inform you.
  • Accounting of Disclosures: You have the right to request an accounting of certain disclosures Denton Vision Center has made of your PHI.
  • Paper Copy of This Notice: You may request a paper copy of this notice at any time.

Changes to This Notice

Denton Vision Center reserves the right to change the terms of this notice and its privacy policies. Changes will apply to all PHI maintained by Denton Vision Center. You will be notified of any changes through appropriate means.

By receiving services from Denton Vision Center, you acknowledge that you have reviewed a copy of this Privacy Notice. You understand that Denton Vision Center’s Privacy Policy is effective immediately starting July 1, 2025.
